Nominations are collected online through the SurveyMonkey portal that will be active only during the Nomination Period as described in the Selections Timeline. Please click here to download instructions on how to register and nominate a student.

We strongly encourage counselors to register with our Program and submit nominations through the SurveyMonkey portal. If you would prefer to fill out a physical form and email it to the Milken Scholars Program, please download the form here.

Eligible Nominees will be emailed an invitation to apply with instructions on how to register for their SurveyMonkey account and complete their application online. Please see the Selections Timeline for the release date of these invitations.
